Implementation of a web based communication system for primary care in RACFs
Central Sydney Division of General Practice (CSDGP) has a long tradition of working as a research partner with local universities to inform and support research in general practice in data collection and implementing interventions. Research expertise and knowledge acquired through this connection is also used in implementing and evaluating our own programs and initiatives.
Currently CSDGP is piloting the implementation of a Virtual Private Network (VPN) model in four Residential Aged Care Facilities (RACFs). A VPN utilises an internet link that enables visiting GPs and pharmacists at an RACF to access their own software on their own practice network. The VPN provides a secure communication channel for GPs and pharmacists visiting RACFs.
In order to better understand the implementation process and the impact of the pilot on the quality of care a collaborative action research approach was chosen for the pilot's evaluation. The evaluation design employs a set of qualitative and quantitative methods such as focus group discussions; interviews with general practitioners, RACF staff, and the pilot coordinator; and a questionnaire to all pilot participants. The evaluation explores benefits to all participants in the area of electronic prescribing, medication management and care planning and provides a comprehensive and in-depth insight in the implementation phase of a new information technology based system in RACFs and GP surgeries. Based on the evaluation results, training programs and a resources manual will be developed in partnership with CSDGP and the participants of the pilot.
The feedback provided by GPs and RACF staff has been very positive and encouraging: “I have been putting all data of every single patient I admit to a nursing home including all of their medications, all of their history up to date in the computer here at the surgery. Now I am able to access that information when I am at the nursing home and write prescriptions and referrals on the spot” reported one of our GPs.
Unlike other models we have seen, the VPN model chosen by CSDGP is simple to implement and simple to use. The research results will inform the roll out of the pilot in the community.
