ROAR is an on-line directory that contains 969 records of current and completed research projects in the area of primary health care; information about primary health care researchers and their research interests; links to research organisations; and information about research funding.

The on-line directory is intended to facilitate networking, collaborations and the sharing of knowledge and we are aiming for an accurate record of Australian research and researchers in the Primary Health Care sector. The following is a selection of the 93 records that were returned using the search terms 'chronic disease management' and 'chronic disease' and 'chronic'.

PROJECTS

A systematic review of chronic disease management

Systematic review will investigate many research questions including:

  • What is meant by chronic disease management in the primary health care sector in Australia and in comparable countries such as USA, Canada, United Kingdom, Netherlands and New Zealand?
  • How and in what context were the models of chronic disease management developed? Why were they developed?
  • In relation to funding, what models of remuneration and incentives encourage improved chronic disease outcomes? How applicable are these to the Australian context and health care system?

A Systematic Review of Models for the Organisation and Financing of Primary Health Care for Mild and Moderate Chronic Lung Disease

This project will undertake a systematic review to identify and synthesise what is known about models for organising and financing primary health care for patients with mild or moderate severity chronic lung disease.

Chronic Disease Management in the Australian Community Setting
DCJ Holman, M Hobbs, JD Emery, DB Preen, CW Kelman, DL Rosman

This project will provide evidence on how best to use the efforts of Australian GPs to obtain better outcomes in patients aged 65+ years who suffer from chronic diseases such as diabetes, heart disease and high blood pressure, asthma and emphysema, seizures and stomach disorders. It will also examine the best way that GP visits can promote healthier ageing in all older seniors, aged 75+ years.
