The Aged Care GP Panel Initiative (ACGPPI) aims to ensure better access to primary medical care for residents of aged care homes and to enable GPs to work with homes on quality improvement strategies for the care of all residents.

Collection of data

The collection of data for the ACGPPI will assist in monitoring the progress of the Initiative and determine the extent to which the Initiative is meeting its objectives.

Divisions of General Practice are required to report on the ACGPPI via two mechanisms:

  1. As part of the NQPS Planning and Reporting Framework in a 6 Month Progress Report for the period ending 31 December, and in a 12 Month Report for the period ending 30 June.  Reporting for the ACGPPI in the 12 Month Report should be a compilation of all information from 1 July to 30 June. See the Residential Aged Care Domain of the National Performance Indicators list for further details of the ACGPPI performance indicators.
  2. Also, the 'ACGPPI Standard Data Items (SDIs) Form' are completed and submitted to each Divisions State/Territory Office. This ACGPPI-SDI form is required from a 6 month period (eg. 1 Jan - 30 June) as this data is collected on a 6 monthly basis.

Divisions and SBOs are encouraged to use the online Planning & Reporting pro forma, however a Microsoft Word based ACGPPI SDI form is also available (see below).
