Program Planning and Reporting Worked Examples

Please note: The Department has entered into new agreements with the Divisions Network which involve a simplification of the complex NQPS implemented for the 2005-08 contract period. From July 2008 to June 2012 Divisions will report on a new set of National Performance Indicators (NPIs). View support pages for this period

 

To help Divisions complete their 6 & 12 Month Reports the Department of Health and Ageing has produced examples of how to report against some of the Indicators and Reflection tables. These have been grouped together by the following areas and relevant examples are available for individual download in an Adobe Acrobat format (PDF).
