Note: Early bird fees must have been paid by early bird cut-off date (14 May 2010) or fees will go up to standard rate.
Please note that As we are in preparation to move onsite, we are no longer accepting on-line registrations. You may complete the Registration Form and fax it to 02 6285 1336 until 1pm on Friday 25 June. After this time you will need to register when you arrive at the Conference.

Cancellation
Delegates who find themselves unable to attend the 2010 PHC Research Conference after submitting a registration form are most welcome to nominate a substitute. All substitutions and cancellations must be made in writing to the Conference Secretariat. Cancellations received before 14 May 2010 will be refunded, less an administration fee of $99. Cancellations after 14 May 2010 are not refundable. Exceptions are at the discretion of the organising committee.
Payment of Fees
All Conference and function payments must be received prior to the Conference. If payment for your registration type (eg. Early bird 14 May 2010) is not received by the cut-off date, your registration type will automatically be changed to the next registration level.
Privacy
Information necessary for your attendance at the Conference will be gathered, stored and disseminated in accordance with the National Privacy Legislation. A participant list with name, organisation, state, email address and mobile number will be supplied to all Conference attendees. If you do not consent to the Conference providing your name, organisation and State for these purposes, please indicate this on the Privacy box on the registration form.
Special Needs
Every effort will be made to ensure people with special needs are catered for. Should you require any specific assistance, catering or arrangements to be made on your behalf, please include a notation on your registration form under the section ‘Special Requirements’.
Insurance
Registration fees do not include personal travel insurance of any kind. It is strongly recommended that, when registering for the Conference and booking travel, a travel insurance policy be taken out to cover loss, cancellation, medical cover etc for any reason. PHC RIS and the Conference Secretariat do not take responsibility for any delegate failing to insure.
