Forgotten your Access Key?
Cancellation Delegates who find themselves unable to attend the Conference after submitting a registration form are most welcome to nominate a substitute. All substitutions and cancellations must be made in writing to the Conference Secretariat. Cancellations received before 30 April 2008 will be refunded, less an administration fee of $99. Cancellations after 30 April 2008 are not refundable. Exceptions are at the discretion of the organising committee.
Payment of Fees
All Conference and function payments must be received prior to the Conference. If payment for your registration type is not received by the cut-off date, your registration type will automatically be changed to the next registration level.
Privacy
Information necessary for your attendance at the Conference will be gathered, stored and disseminated in accordance with the National Privacy Legislation. A participant list with name, organisation and state will be supplied to all Conference attendees. If you do not consent to the Conference providing your name, organisation and State for these purposes, please indicate this on the Privacy box on the registration form.
Special Needs
Every effort will be made to ensure people with special needs are catered for. Should you require any specific assistance, catering or arrangements to be made on your behalf, please include a notation on your registration form under the section “Special Requirements”.
Insurance
Registration fees do not include personal travel insurance of any kind. It is strongly recommended that, when registering for the Conference and booking travel, a travel insurance policy be taken out to cover loss, cancellation, medical cover etc for any reason. PHC RIS and Conference Secretariat do not take responsibility for any delegate failing to insure.
